Different ways to score adherence questions

Hi!
I have used ACTG questions to ask patients about adherence:
 - missed pill last week-end
 - how often spend >1 day without pills at all in pill box
 - missed any pharmacy visit last 6 months
    - if yes, how many visits
 - missed any clinic visit last 6 months
    - if yes, how many visits
 - When was the last time you missed taking any of your ARVs at all.

I know Reynolds N et.al. suggested a formula to use, but I didn't
cover all the questions in that formula.
I scored the questions as 0 good adherence up to highest number
(depends on answers for each question) and a high number will be poor
adherence. Tried to use median level but not sure it will be useful?

Any tips or suggesting any refs will be much appreciated.
