Non-adherence in PMTCT (SD-NVP)

Hi all,
Another study on non-adherence in PMTCT (SD-NVP) was published recently (Kuonza et al 2010) noting, among other things, the association between Non-adherence to the infant dose of nevirapine and home deliveries, and between improved maternal adherence with, among other factors, giving the mother a NVP tablet to take home during antenatal care (ANC). There are a lot of factors associated with adherence and non-adherence and many of them are social in nature (education levels, disclosure to partner etc.) but it seems like one of the issues that we keep encountering is the encouragement/discouragement of home deliveries and ANC attendance, and their affect on adherence. Do you have any experience with programs tackling the issue in a hospital-community setting?
