Rapid Implementation of the Xpert MTB/RIF diagnostic test. Technical and operational ‘How-to’ Practical considerations. April 2011

This document was prepared by a small Writing Group under the coordination of the TB Diagnostics & Laboratory Strengthening unit of the Stop TB Department (STB) of WHO, with contributions and support from several Working Groups of the Stop TB Partnership. The contributions of the following Working Groups and their respective secretariats are especially acknowledged: Global Laboratory Initiative (GLI), MDR-TB, TB/HIV, and DOTS Expansion. This document is a product of the discussions and
consensus reached during the Global Consultation on implementation and scale-up of the Xpert MTB/RIF system, convened by WHO-STB in early December 2010, and subsequent active interaction by the various Working Groups and their secretariats.
Data underlying the rapid implementation of Xpert MTB/RIF were kindly provided by FIND (Foundation for Innovative New Diagnostics) to WHO. FIND will continue to play a key role in Xpert MTB/RIF implementation and roll-out, and will assist with monitoring of global sales, post-marketing surveillance, and technical assistance to countries.

These documents introduce major changes in the policy and management of TB patients from case definition to diagnostic algorithms to user training are now required with the implementation of the Xpert MTB/RIF technology. In an effort to facilitate these changes, the Stop TB Department at the WHO has written documents that outline policy action items needed for the successful implementation and roll-out of Xpert MTB/RIF. The first document highlights the outcomes from the Global Consultation in December 2010 and discusses a timeline moving forward for the guidance documents, workshops, meetings, and consultations. The second and third documents provide a more detailed road map for the role out of the new technology identifying how to use the test, risk management, pricing, and strategic planning for phased implementation.
