Treatment as Prevention: To Be or Not to Be?

Is Treatment as Prevention Realistic Among Serodiscordant Couples?

Although mathematical models suggest that treatment as HIV prevention holds promise, concerns remain. Using the Wang et al. study as an example, Cohen questions the practicality of treatment as prevention. According to this retrospective study of 1,927 discordant couples, 4.3% of the HIV-negative partners seroconverted, with the rate of seroconversion increasing over time, whether or not the index partner received access to counseling, condoms, and free antiretroviral therapy.

Effective Measures to Packaging PrEP for Prevention of HIV

The promising effects of topical and oral pre-exposure prophylaxis (PrEP) may be undercut by low uptake, suboptimal treatment adherence, and increased HIV risk behavior among PrEP users. To address these concerns, the authors provide a five-part strategy, including PrEP prescription, safety screening, behavioral intervention, integration of PrEP with other health care services, and population-level monitoring.
