Thank you very much for this enlightening but scarry video. I am thinking
of the cost involved, USD50-100000. its a lot/ here is a testimony from a
Health care worker. This is really an eye opener and as a TB culture lab
scientist, I am asking myself a lot of questions.

This is just an absolutely excellent video and should be widely publicised- Healthcare workers are at great risk in all the high burden countries including SA. While there has been training, teaching, improving PPE supplies, there are several challenges at clinical level to carry out proper IPC at a basic level. It was so unfortunate that Darlene contracted MDR-TB, she and Arne are so brave and also make great ambassadors to talk to healthcare workers- some of whom have a very blasé attitude towards TB despite considerable training and resources used in containing TB. This unfortunate episode has motivated much activity by these wonderful young doctors to educate medical students- that is marvellous and well done! I admire both of you

I just want thank those two South African doctors for sharing their
personal experience with MDR-TB. It is the problem what healthcare workers
are facing everyday especially in TB endemic areas. The video makes it real
to policy makers and others who may not provide direct care to patients.
It's also frustrating to see how slow the pharmaceutical companies have
been in developing new drugs. It's time to focus on TB which is a major
health problem for so many people around the world.

I had a chance to talk with Arne at length at the Kuala Lumpur UNION
meeting where they also presented. One issue is that it is not common in
South Africa to offer treatment for latent infection for those definitely
exposed and almost certainly infected with MDR TB - without active disease.
While there are no clear guidelines, the principle of treating latent
infection rather than waiting for reactivation disease is well established
and there are now several treatment options other than the "wait and see"
approach. It is much more difficult to treat active disease than to prevent
it, in principle. I would urge the S. African officials and other high
MDR setting to develop a policy for MDR TB prophylaxis. In fact, the
situation is such that a clinical trial in the RSA could be done in high
risk groups, informing the world on the best options. Pediatric and
HIV-infected close contacts are the other options. In an editorial with
Gavin Churchyard and Jerry Friedland, we argued that the new drugs ought to
be considered for clinical trials, also informing drug developers of their
action alone or in combination against latent, or persister-like organisms.
There is, as always, politics involved. In a world where only a fraction
of the MDR TB is diagnosed and treated, what is the role of treating LTBI?
However, in my opinion, cases like Arne's make a compelling argument for
developing guidelines, and possibly, for controlled clinical trials.

Totally agree, Ed. And we need accountability for TB-IPC implementation. While there has been much training, provision and support, the shortage of staff, and frankly, burn out is so apparent, that it is impossible to get HCW to actually believe there is a huge risk in the daily place of work. Despite all the time and effort spent, why do we still have crowded outpatient and waiting areas? Why are we not following FAST frequently enough? Why are we not looking at anti-TB sens patterns within 3 days? And most importantly, we should be supporting our HCW though this dreadful ordeal.

I agree - health providers truly imagine themselves to be immune. Or they carry a strange mixture of bravado and nihilism. I guess when there are no masks available on the TB wards or the medical wards in the highest prevalence areas you adopt these attitudes to survive psychologically. As visitors we got into the same mind set - thank you for an excellent reminder of our vulnerability.

"This is just an absolutely excellent video and should be widely publicised- Healthcare workers are at great risk in all the high burden countries including SA. While there has been training, teaching, improving PPE supplies, there are several challenges at clinical level to carry out proper IPC at a basic level. It was so unfortunate that Darlene contracted MDR-TB, she and Arne are so brave and also make great ambassadors to talk to healthcare workers- some of whom have a very blasé attitude towards TB despite considerable training and resources used in containing TB. This unfortunate episode has motivated much activity by these wonderful young doctors to educate medical students- that is marvellous and well done! I admire both of you

This video is a must watch for everyone including policy makers! I salute
Arne and Dalene for this action of making this testimony available
for every one to watch and understand the gravity of TB problem and most
especially drug resistant TB. The video discusses a range of issues from
mild to more agonizing side effects, investigation and social support
challenges plus and need for timely interventions among others. Personally,
I have been coordinating a National MDR-TB Program but when you talk to
resource managers and policy makers to make provisions to support MDR-TB
control, they usually don,t appreciate the problem. This video
is definitely going to support our efforts. The other thing I need to bring
out here is the danger of getting exposed to *unrecognized TB* to which
all of us are vulnerable. Therefore, there is need to have good TB
infection control practices in all TB risky places (especially congregate
settings). This should bring about identification, separation,
investigation and treatment of confirmed cases in real time and space.
Further, more there is need to have occupation health policies in place
supporting compensation of affected workers. Health should not be traded
like produce! It is life and we need to take care of every one. Drs Arne
and Dalene once again thank you very much.

A must watch for all HCW through out the world. Its also scary . We in city of Mumbai looks are sitting on timebomb waiting to explode if corrective and preventive measures not taken seriously.

TB PROOF was founded in 2012 by South African Health Care Workers and Students after multiple personal experiences with occupational tuberculosis, particularly multi-drug-resistant TB. This initiative has three main goals:

To “TB PROOF” our colleagues, students and patients by creating greater awareness of and protection against occupational and nosocomial TB transmission through education and activism.
To destigmatize and prioritize occupational TB by collecting and disclosing proof of an alarming rise in cases.
To mobilize national and global resources through patient advocacy to help address shortcomings in prevention and treatment strategies, both for health care workers and more importantly, the population we serve.
