Position: Population Manager
Location: Brigham and Women’s Advanced Primary Care Associates, South Huntington
Jamaica Plain, MA

Brigham and Women's Advanced Primary Care Associates, South Huntington is an internal medicine practice opening in the summer of 2011. Our goal is to create an environment that maximally fosters healing relationships between patients and their health care team, while supporting an outstanding educational experience for future members of that healthcare team. The practice will test innovative models of primary care delivery. It will be continually improving in a way that is accessible to being evaluated and disseminated. The practice will be certifiable at the highest level as a Patient-centered Medical Home.

We have created a position for a population manager to create and manage patient registries and to provide the members of the health care teams the data they need to meet the health needs of the local community and contribute to improved population health. This individual will support practice staff to develop creative processes to proactively manage chronic disease.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

• Patient Registries
o Adapt, or when necessary create, databases to function as patient registries. (Databases that include key information on important patient groups within the practice’s population.)
o Monitor the database to identify and reach out to those needing healthcare
o Automate and perform processes to send letters and e-mails to patients either overdue for medical care, or out of compliance with the goals for their care
o Generate lists of patients to contact by phone, as well as phone scripts for use by staff making the call
o Meet weekly with each care team to help create plans for specific patients and populations not achieving optimal care
• Quality Improvement and Process Design
o Collaborate and provide data to care teams to establish population-appropriate pre-visit and point of care processes
o Gather and collate data to be used in rapid cycle improvement
o Create, maintain and distribute results of key metrics around quality, access, patient experience and value to leadership, staff, and patients
• Knowledge Retention and Disbursement
o Provide technical support for quality improvement activities, specifically providing relevant staff with the necessary tools and approaches to improve quality
o Assist staff on methods of improvement, metric selection, data collection, and analysis
o Create documentation needed for recognition of the practice as a Patient-Centered Medical Home
QUALIFICATIONS:
• Bachelors degree required
• Proficient in data management, manipulation, and reporting; must demonstrate capacity to work with large datasets with guidance of medical professional
• Advanced knowledge of Microsoft Office, including MS Access and Excel
• Proficient knowledge of data manipulation and querying tools, including SQL, SAS, or STATA
• Experience with report design in Excel or other reporting tools
• Proficient with MS Word and Outlook

• Spanish language skills a plus but not required
• Experience with LEAN or other forms of rapid cycle improvement a plus, but not required.
