RFI on Health IT for Community Health Promotion (BP Focus)

Hi, I'm writing from the New York City Department of Health's Primary Care Information Project. Over the last year, we've been in discussion with a group of community organizations (churches, senior centers) all of whom offer their members regular blood pressure monitoring, as well as other health promotion programs with a chronic disease focus. They currently use paper blood pressure tracking cards and are looking for a tool to make the information more actionable for outreach and population health analysis. The RFI is to allow us to identify a health IT system that would allow their members to securely keep track of their BP readings and to share their information with the community health worker in charge of the program.
I'm posting this here to spread word of the RFI to any vendor/organizations that might respond and also to get your thoughts on the most usable and effective health IT systems for community health workers. Our vendor experience has been constrained to the domestic market.
