About Carole Mitnick, ScD
Carole Mitnick, ScD received both her ScM and ScD in International Health Epidemiology and Ecology from the Harvard School of Public Health in Boston. She began working with Partners In Health in 1996 when she was a Fellow in Health and Social Justice. Dr. Mitnick was appointed an Instructor in the Department of Global Health and Social Medicine at Harvard Medical School in 2002. Her primary research centers on clinical outcomes research on multidrug-resistant tuberculosis (MDR-TB), particularly in resource-poor communities. Dr. Mitnick's current projects involve evaluating treatment protocols using novel analytic approaches and performing clinical trials of MDR-TB. She is also working to identify surrogate endpoints for MDR-TB therapy and to assess the effect of quality improvement initiatives on TB treatment outcomes.

    Thanks, all. I agree with the discussion overall and the importance of separating the guidelines on clinical decision-making from those on reporting. This was one of the questions that proved most challenging when we first were working on the consensus definitions. We tried to come up with different scenarios depending on when the interruption occurred (first or second year) and on culture/smear status at reinitiation.     Michelle- I agree with Irina's comments below. Regarding the specific patient you mention in your response, i don't think there is any specific guidance offered in the Guidelines or in Laserson et al, IJTLD, 2005. These were both written under the assumption that sputum samples wouldn't be collected at the frequency at which you're collecting them.
