Hi all Kelly - if you discover such a resource please let us know - we have struggled with how to do panel proficiency testing for MODS which uses sputa not strains to give direst RH DST - I have heard of somewhere in the US that can provide simulated sputa but as I recall it cost hundreds (if not thousands) of dollars, and we have not identified a straightforward way to do it - ...

Congratulations to all for a fascinating discussion. A point of clarification regarding the use of cording as confirmation of MTB in liquid cultures (comment 38). Using MGIT cultures in South Africa Chihota et al (IJTLD 2010; 14 (8): 1024-1031) demonstrated that the sensitivity and specificity of demonstration of cording for identification of MTB was identical to the use of MPB64 testing (99.0% and 97.9% respectively) confirming the findings of all groups who have published their ...

We would be very interested to hear of the experiences of others in the use of therapeutic drug monitoring (TDM) in management of MDRTB. Specifically we are interested to hear of any known evidence supporting the use of higher peak amikacin levels than one would usually use (the "plasma level>=10x MIC" approach). Do others have experience or insights into how much this is science and how much it is educated conjecture?

This query about aminoglycosides brings me back to my earlier question in July about therapeutic drug monitoring. I thank Yue Guan for the response on behalf of Charles Peloquin but I am still unclear about the science behind the increased target levels of amikacin (which seem to cause such trouble).
