About Irina Gelmanova, MD
Dr. Gelmanova is a Project Consultant to Partners In Health’s DOTS-Plus program in Tomsk oblast. She also serves as a Research Committee member for the PIH TB services and MDR TB project in Tomsk. Prior to assuming these roles, she worked in PIH’s Moscow office as Project Coordinator of the Tomsk oblast MDR TB Project. Dr. Gelmanova also has experience working in the pharmaceutical industry and spent eight months coordinating a pilot TB project with Médecins Sans Frontières in Kazakstan.

Dr. Gelmanova received her MD from Almaty State Medical Institute in Almaty, Kazakhstan. She completed her Masters in Public Health at Boston University School of Public Health.

    This article provides hope that XDR TB cases can be treated successfully. 60.4% success is a great result. However, in Tomsk we treated successfully only 48.3% XDR TB cases as compared to 63.2% of non-XDR TB cases. Maybe we should compare Peruvian and Tomsk XDR TB cohorts regarding any differences in resistance patterns and other important determinants of failure/success.     Hi Nida, I think most of the projects choose treatment supporters among neighbors/previous patients, so they are also "buddies". There might be a need in a separate peer-support system in places where DOT is provided by medical staff. There are comprehensive manuals regarding peer support for LTBI, which might be interesting to you.     Hi Mamel, Salmaan has already mentioned our challenges with "courses after default" in Russia There is a table with recommendations for clinicians how to deal with returning defaulters from MDR TB treatment.     Dear Marina, Thank you for your note. Athough I am not a laboratory person I was interested in your reply. I think it is not a rare situation when we have to keep an outreach laboratory but the quality goes down due to low volume of tests. I would appreciate if you provide more insight regarding "additional steps to ensure laboratory proficiency" in laboratories with low volume of tests. Thank you, Irina
