Patients' record mobile app?

I am searching for mobile app to tecord data of my patients. I prefer it to be free with cloud sync.
Can you help me please?

Dear Dr. Malhady,

An app that I developed that measures an intervention for any clinical population and is a complete electronica record taking into account morbidity, concurrent treatments and life events. It also provides a basis for learning assessment.

It was recently reviewed in the journal Family Medicine. Below is the introduction and a link to that review Based on a recent book about the learning component of the method:

The application itself went stale and we are in the process of updating it and mounting it on I tunes.

The application operates On an iPad and it’s not presently designed for other devices.

If you find that it is suitable, I can send you a copy of the app (ipa) but you would have to install manually on an iPad that had the pre-November 2018 iOS. There are instructions for this on YouTube.
